• Learning Contract
• Parental Permission and Release Form
• Legislative Information Form
• Personal Information Form
• Copy of Insurance Card – FRONT & BACK
• Up-To-Date Immunization Certificate – CHECK THE EXPIRATION DATE
• Damage Deposit for $25.00
• Self addressed stamped envelope
